Nurses who practice in all health care specialties and environments – from hospitals and clinics to home care, schools, hospice, private practice, research, education and more, can be nominated. Winners will be recognized in the December 2012 issue of Madison Magazine and honored at a special awards program.

In 300 words or less, explain why this nurse deserves recognition. The best nominations share a story or instance where this nurse’s action(s) went beyond the call of duty while caring for a patient, assisting a patient’s family and loved ones, or serving his or her colleagues in a notable way.
- Nominee must have been trained in a formal nursing program
- Nominee cannot be in active disciplinary action
- Applicants may be vetted against Wisconsin Board of Nursing records or any other applicable regulation and licensing oversight (ex. Nursing Home Administrator Examining Board, Midwives Advisory Committee, etc.)
- Winners will be chosen based on a variety of editorial criteria, including nursing specialty (Madison Magazine is looking for a nice mixture of health care environments in which nurses practice), professional experience (from those just starting out in the field to accomplished veterans) and quality of the nominations.
