Meriter’s Intensive Outpatient Rehab Program
offers a comprehensive and interdisciplinary approach to rehabilitation for individuals who have experienced functional changes resulting from neurological impairments and pathologies.  This intensive program is a half-day service delivery model, with interdisciplinary therapy services provided as frequently as five days a week. 

Patient diagnosis may include:

  • Stroke
  • Traumatic or anoxic brain injury
  • Spinal cord injury
  • Multiple orthopedic injuries
  • Amputation
  • Neurologic disease 

Our interdisciplinary team provides quality rehabilitative services and is recognized for their expertise and advanced certification across a variety of specialty areas.  This integrated team meets regularly to discuss individual patient needs and treatment plans.  Patients and family members work closely with the team to establish patient-centered goals and promote independence and adjustment to disability.  Team members include:

  • Physical Therapy
  • Occupational Therapy
  • Speech Therapy
  • Rehabilitation Medicine
  • Rehabilitation Psychology
  • Case Management

  Eligibility for the Intensive Outpatient Rehab Program:

  • Require two or more of the following therapies:  Occupational, Physical and Speech Therapy
  • Motivated and able to participate in a half-day intensive therapy program 3-5 times per week.
  • Medically stable.
  • 15 years of age or older.
  • Good potential to achieve functional gains.
  • Do not require staff supervision between sessions or for using the bathroom.  However, if supervision can be provided by a caretaker or family member, we may be able to accept the patient into the program.   

Working together, our goal is to help patients regain the confidence and skills to resume their former life roles, explore new resources and develop a plan to reach the highest possible level of independence.
