Meriter Pediatric Clinics Now Offer MyChart
If your child receives care at a Meriter Pediatric clinic and you would like to access portions of your child's medical information online, then download the MyChart Proxy form below and mail or fax it in (address and fax number are located on the form). Once we receive the request, your personal activation code will be mailed to your home within 7 to 10 business days. Learn more about the features and benefits of Meriter MyChart.
Online Bill Pay
You now have the option to pay your clinic bill online! Meriter's online bill pay service allows you to securely submit payment for your health care bill in the most efficient manner possible - online with your credit card. To pay your clinic bill, go to meriter.com/billing.
Prescription Refills
If you need a prescription refilled, we ask that you call the clinic and leave a message. Please include the following information:
• Your child's name
• Your phone number
• Your child's date of birth
• Name of the pharmacy
• Amount and frequency of the medicine
We will call in your refill order to the pharmacy you designate within 24 hours, and notify you if there are circumstances that will not allow it to be refilled.
