Choosing a home health provider for yourself or a family member can be a difficult decision. Before selecting an agency, you may want to ask the following questions:

Q. How long has this provider been serving the community?

A. Meriter Home Health was founded in 1985 and is affiliated with Meriter Hospital, a cornerstone of the southcentral Wisconsin health care community for more than 100 years.

Q. Is the provider accredited through the Joint Commission?
A. The Joint Commission is responsible for ensuring quality in healthcare organizations. Participation in this rigorous evaluation and survey is voluntary. Q. Is the provider's skilled nursing care covered by Medicare and Medicaid?
A. Home health care is among the many benefits offered by Medicare and Medicaid. Learn whether an agency is an approved provider by asking your physician, hospital discharge planner, community referral source or the home health provider directly. Q. What is the provider's reputation in the community?
A. Ask your physician, hospital discharge planner or other healthcare providers for a listing of home health providers in your community. Recommendations from friends and family members that have received home care services can also provide guidance. Meriter Home Health has an excellent reputation, and consistently receives high marks for service and quality care from its customers.

Q. Does the provider offer comprehensive home care services?
A. Choosing a comprehensive provider will assure you and your family convenience and continuity of care. Meriter Home Health offers a full array of home care services, including skilled nursing care, therapy services, infusion therapy, respiratory care, home medical equipment and supplies. Q. What emergency services are available?
A. You should choose a provider that guarantees availability of care and services. Meriter Home Health's staff is available 24 hours a day, 7 days a week.

Q. Do the home care professionals educate and partner with family members and other caregivers?

A. Successful home care plans are built on a partnership foundation of home care provider, client and caregivers. Meriter Home Health works closely with clients and caregivers to develop a care and service plan that meets the needs of each client and family. Meriter Home Health is committed to providing caregiver education and support to our patients.

Q. Does the home care provider stay in regular contact with each patient's physician?
A. Meriter Home Health works with your physician to ensure that you receive the right services to meet your medical needs. Our staff provides your physicians with progress reports and written communication throughout your care and treatment.

Q. Does the agency assist each family in determining their insurance coverage and service eligibility?
A. It is important that you share all insurance information that you have with your provider. The provider should notify you of the charges associated with your products and care at delivery. Insurance coverage information and eligibility for benefits will be outlined by the provider based on information that they have been able to gather from your payer.

Meriter Home Health's customer service staff is prepared to assist you in determining insurance coverage and eligibility for services. Additionally, many insurance companies and HMO's have established preferred-provider contracts that may direct your ability to purchase services from some providers.
