Please call 608.417.6447 to make an appointment for a heart health risk assessment or to find out more about the program. You do not need a referral from your primary care provider to receive care from the Women's HeartCare Program.
Insurance
The costs of Meriter Women’s HeartCare’s risk assessment and lifestyle intervention program are covered by major health insurance plans. Please check with your individual carrier about deductibles or co-pays for which you may be responsible.
