Discharge planning begins at admission. Your healthcare team (physicians, nurses, care coordinator, therapist, social worker and others) will work with you and your family to ensure that your discharge is safe and timely. The team will help you assess your needs, and will provide support, information and referrals to make sure that you can function as independently as possible after your discharge. If needed, the team can help identify potential sources of financial aid and additional resources. Your questions about treatment and continuing care are important. If we don’t completely answer them, please tell us.
Discharge
Your doctor will determine when you are ready to leave the hospital. Your healthcare team will make arrangements for your discharge and will give you home instructions before you lave. If you have any questions, please ask us.
Standard Checkout Time
Medical / Surgical Units: 11 a.m.
Obstetrics: 1 p.m.
Please make arrangements for your transportation to be at the hospital by the standard checkout time, if at all possible. A nursing staff member will accompany you to your transportation. Before you leave, check to be sure that you have all your belongings with you.
