Overview
Meriter uses a web-based electronic protocol submission, review
and communication
system.
Through your electronic user account, you will:
-
Complete all Meriter forms (Initial Reviews, Continuing
Reviews, Protocol Changes or Amendments, Study Closure,
etc.)
-
Submit all research-related documents
-
Correspond with the IRB
How Do I Request a User Account?
Please note that if you create a duplicate account, you will be
contacted by IRB staff. If you can't remember if you have a user
account, please contact the IRB office at (608)
417-6411.
To request an account:
1. Visit our electronic site's
welcome page.
2. Click the "Request New Account" link in the blue bar at the
bottom of the page.
3. Complete the information required.
Accounts are set up during regular business hours, Monday -
Friday. You will receive an e-mail notification in 24-72
hours when your account
has been activated. For quicker account activation, call the IRB
office at (608) 417-6411 and leave a message that you have
requested an account.
Who Needs a User Account?
All research staff that has contact with Meriter patients,
patient records or patient specimens for research purposes.
These are considered Key Study Personnel for your study
protocol. They will need to fulfill the Meriter Human Subject
Training Requirement. Compliance with this requirement is
tracked in the online submission program.
The electronic system blocks your protocol from being
submitted if there are any key study personnel who
don't have a current training certificate linked to their user
account. Your protocol will not reach the IRB to be reviewed.
You will not receive initial or continuing approval. Your
protocol will expire and be terminated.
How Does My Training Certificate Get Linked to My User
Account?
Send an electronic or fax version of your certificate to the
IRB Office. The IRB Coordinator links certificates to user
accounts. Electronic versions can be sent to
lmichaels@meriter.com. Faxes can be sent to (608)
417-5934.
Do I Need to Make Hard Copies of My Application?
Initial Full Board Reviews – 12 collated copies
of all submission materials from the electronic system
delivered/ mailed to the Meriter IRB Office at 1 Center, Meriter
Hospital.
Major Modifications – 12 copies of all submission
materials from the electronic system delivered/mailed to the
Meriter IRB Office at 1 Center, Meriter Hospital.
Initial and Continuing Expedited Reviews – NO
hard copies
Reviews for Exemption – NO hard copies
Full Board Continuing Reviews – NO hard copies
Protocol Changes – NO hard copies
Study Closure Report – NO hard copies
