About Our Program
Each year, Meriter Hospital seeks qualified candidates for our Pharmacy Practice Residency.
The Meriter Hospital Pharmacy Practice Residency is a one-year, full-time, post-graduate experience accredited by the American Society of Health-System Pharmacists. The goal of the program is to guide the Resident in developing advanced knowledge and skills in clinical areas of contemporary pharmacy practice and in providing comprehensive pharmaceutical care. The Resident will apply this expertise in a community hospital setting.
The Pharmacy Practice Residency has two main objectives:
1. Provide the Resident with a broad scope of in-depth experiences that will:
- Lead to an advanced level of knowledge and pharmacy practice skills
- Enhance the Resident's ability to design, implement and improve clinical pharmacy services
- Increase the Resident's experiential and didactic teaching skills
2. Develop competent clinicians who:
- Have an understanding of the concepts used in rational drug therapy
- Have the ability to utilize and expand their expertise in clinical pharmacy practice, education and research
- Provide comprehensive pharmaceutical care to diverse populations of patients
Meeting these goals and objectives will be facilitated by:
- Working with a variety of experienced preceptors in core and elective clinical pharmacy rotations
- Actively participating in the pharmaceutical care of patients affiliated with our institution
- Participating in interdisciplinary clinical practice
- Exercising independent motivation and initiative
- Presenting seminars/cases/lectures
- Co-precepting doctor of pharmacy students
- Teaching pharmacy students from the University of Wisconsin School of Pharmacy in their pharmacotherapy labs
- Attending educational conferences
- Completing clinical projects and one major research project
- Flexible scheduling and ability to design the residency to meet individual needs and goals
- Guiding professional development of the resident with daily feedback and regularly scheduled discussions with preceptors
- Participating in a teaching certificate program coordinated by Meriter Pharmacy staff and the University of Wisconsin School of Pharmacy staff
- Presenting your research project at the Great Lakes Pharmacy Residency Conference

How to Apply
First, print out the Pharmacy Residency Application. (PDF file) Interested applicants must:
- Be graduates of an ACPE-accredited College of Pharmacy
- Participate in the ASHP Resident Matching Program
- Submit all materials that are listed on the application form
- Be eligible for licensure to practice pharmacy in the State of Wisconsin
Please note that we conduct on-site interviews every year in January and February.

The Residency Experience
The Meriter Pharmacy Practice Residency allows significant flexibility for Residents to individualize their training to meet their own specific clinical needs.
Clinical Pharmacy Experiences
Core residency experiences:
- Orientation
- Internal medicine
- Drug information/drug-use policy development
- Practice management
- Critical care
- Infectious disease
- Nutrition support
- Pharmacotherapy lab
Elective residency experiences that can be selected from, but are not limited to:
- Pharmacy administration
- Neonatalogy
- Cardiology
- Pain management
- Psychiatry
- Pediatrics
- Emergency medicine
- Oncology
- Surgery
The Resident and Program Director select elective rotations to enhance or complement core experiences. Electives are generally scheduled in the second half of the residency year to allow time for the Resident to develop areas of interest and to become familiar with the program and the hospital. In some circumstances, it may be possible to arrange for specific experiences at other teaching sites.
The length of each experience may vary according to the interest of the individual and the time(s) available. Examples of rotation lengths include: 4 weeks; 6 weeks; 8 weeks sequentially or distinctly (4 wks x 2); longitudinal (for example, practice management is 0.5-1 day/week over the course of the year).
Selection and scheduling of these experiences will be done at least semi-annually by the Program Director following discussions with the Resident and the individual preceptors. Semi-annual scheduling allows for flexibility within the program to meet the individual needs of each resident.
Preceptors
Residency preceptors are actively involved in educational programs and endeavors at Meriter Hospital. Further, many members of the department are engaged in scholarly activities and participate regularly in professional organizations at regional, state and national levels. A teaching affiliation with the University of Wisconsin School of Pharmacy enhances the academic opportunities of clinical pharmacy training.
Staffing (Service Commitment)
Due to the intrinsic role of pharmacy practice in pharmaceutical care, the residency experience will include weekend staffing. The staffing schedule will be approximately every other weekend (20 hours staffed). The sites of staffing may include specialty service, central or other clinical service coverage. In addition, the Resident may provide patient care on assigned units as part of a scheduled rotation.
Research Project
While we anticipate that the Resident will be included in many projects throughout the year, the Resident is required to be primarily responsible for at least one major project. The goal of this assignment is to gain experience in various aspects of research methodology. The objectives of the major project include defining a research idea, protocol development, submission to the Institutional Review Board, data collection, data analysis and presentation/publication of results. These five objectives can be accomplished within one project, or from various projects throughout the year. The Resident is required to present their project at the Great Lakes Pharmacy Residency Conference which occurs at the end of April each year. Examples of past Resident projects include:
- Sedation Protocol for Medical Imaging Procedures in Pediatric Patients
- Evaluation of dinoprostone vaginal insert and extemporaneously compounded PGE2 gel for cervical ripening
- Assessing the impact of transferring responsibility of aminoglycoside monitoring from a pharmacokinetics consult team to all staff pharmacists
- Design and implementation of a teaching certificate program for pharmacy residents
- Ciprofloxacin, levofloxacin,gatifloxacin, and moxifloxacin activity against Pseudomonas aeruginosa at various inocula using E-test method
- Evaluation of enoxaparin associated retroperitoneal, abdominal, and pelvic hematomas
- Evaluation of hyperlipidemia and LDL goal attainment in a cardiology clinic
Assessment and Evaluation
The strength of our program is greatly dependent on feedback and communication. Each rotation begins with a discussion of individualized goals and objectives for the rotation from both the Resident and preceptor. The preceptor will also discuss the learning activities that are incorporated into each rotation.
The residency program utilizes the Residency Learning System (RLS) model for evaluation and feedback for the residents. Components of the RLS model include the following:
- Mid-rotation evaluation (by preceptor)
- Criteria-based checklist evaluations (by both preceptor and resident)
- Final evaluation of Resident (by preceptor)
- Preceptor evaluation (by Resident)
- Resident self-evaluation
Documentation of clinical pharmacy experiences will be completed no later than 2 days after the end of a rotation.
The Resident and preceptor will discuss the evaluations and then submit these forms to the Program Director. The Resident will meet with the Program Director no less than quarterly to discuss progress, and develop goals and plans for the next quarter.
Certificate
The Resident will be awarded a Residency Program certificate upon successful completion of the goals for the program.
