Things are starting to look a little different around here…

Meriter – UnityPoint Health is excited to bring you a new MyChart experience! You now have access to MyUnityPoint, a version of MyChart used by our award-winning colleagues throughout UnityPoint Health.

The layout of MyUnityPoint may be a little different than what you’re used to using, but all your favorite features will remain. Along with our community connect partners at Associated Physicians and Madison Women’s Health; we hope to bring you even more great features in the near future to improve your care experience!

We are honored to partner with you for your care now and into the future.

How do I get started?

If you don't have a MyChart account yet, contact your clinic directly or click the Sign In button below to request an account or enter your activation code.

If you already have a MyChart account, there is nothing you need to do. Your health records for the last three years, along with existing access to other accounts, like your child, spouse or partner’s, will transfer automatically when MyChart changes as of September 13.

In almost all cases, you log-in information will also transfer. If we need to update your log-in information, we will contact you in advance. 



We understand you may have questions about this change. We tried to answer many of them here.

*As we populate your previous test results and medical history into your new MyChart account, you may have erroneously received an alert that you have new tests results. We apologize for this inconvenience and are working diligently to have this corrected to ensure you’re only contacted when you have a new result added to your account. Please know we are continuing to add your previous health history to your MyChart, as well.*
