Meriter is proud to have the opportunity to provide our community with
healthcare services at our Middleton clinic. The clinic is staffed by six
physicians who are board-certified in internal medicine. Pictured left to right

We designed Meriter Medical Clinic-Middleton with patient comfort and privacy
in mind, and trust that you will find comfort in our consistency and value our

Our physicians and staff know that in order to treat the whole person, they need
to be partners in health - keeping you and your family well, encouraging healthy
lifestyle habits and treating you when you're sick. We provide personalized care
and, if necessary, will refer you to medical specialists who we feel will
provide the same level of care you have come to expect from us.

Also, you'll need to send in an Authorization/Consent for Release of Medical
Information form and a Patient Registration form. These forms can be found in

Services
It makes sense to provide care in one place. Your decision to see an
internist is common practice in the United States, where nearly 60 percent of
doctor visits in any given year involve primary care specialists.
Meriter Medical Clinic's primary care specialists have important roles in
healthcare, and want you to feel confident that we manage your care well. They
serve as personal care physicians, provide preventive and diagnostic services,
counsel and educate patients on appropriate health behaviors, manage and
coordinate patient care and facilitate access to specialty and follow-up care.
Meriter Medical Clinic offers:
- General medical exams
- Prenatal exams or pap tests
- Diagnostic and screening services
- Radiology services, including general x-rays

J. Evan Blanchard, MD
Dr. Blanchard is a graduate of University of Nebraska Medical Center in Omaha,
and served his internship and residency at UW Health Hospitals & Clinics. He has
practiced medicine in Madison since 1991. He is board certified in internal medicine, and has a special interest in wilderness medicine.

Mark F. Davis, MD
Dr. Davis is a graduate of University of Arizona Medical School, and served his
internship and residency at UW Health Hospitals & Clinics. He is board certified
in internal medicine, and has a special interest in preventive health. |

Kenneth Felz, MD
Dr. Felz is a graduate of Ohio State University College of Medicine in Columbus, Ohio, and served his internship and residency at the University of Vermont, Mary Fletcher Hospital. He is board certified in internal medicine, and has special interest in treating a full spectrum of adult internal medicine problems, including diabetes management, cardiac problems, gastrointestinal issues, sports medicine and preventive health. |

Irene Mirkin, MD
Dr. Mirkin is a graduate of Ohio State University College of Medicine in Columbus, Ohio, where she also served her internal medicine residency. Dr. Mirkin also served in the Epidemic Intelligence Service at the Center for Disease Control, an assignment that brought her to Madison, where she worked at the state health department in chronic disease epidemiology. Dr. Mirkin is board certified in internal medicine, enjoys caring for adult patients in all aspects of acute and chronic disease, and is particularly interested in preventive medicine. |

Dr. Nemovitz is a graduate of UW-Madison Medical School and served his internship and residency at Gundersen Clinic and Lutheran Hospital in La Crosse, Wisconsin. Dr. Nemovitz is board certified in internal medicine, and has a special interest in preventive medicine. |

Dr. Nestleroad is a graduate of the University of Missouri-Columbia Medical School, and served his internship and residency at UW Hospitals and Clinics. He is board certified in internal medicine, and has a special interest in preventive medicine. |
Making Appointments
There is never a good time for an illness to arise or an injury to occur. Our
staff strives to take the frustration out of appointment scheduling by
accommodating your specific needs. We strive to stay on schedule, because we
know how valuable your time is.
In order to be able to schedule ill patients on the same day they call the
clinic, we hold open several appointment slots. If you feel you need to be seen
on an urgent basis, we will likely ask you to speak with a clinical staff person
who will assess your need and arrange for appropriate care.
If you are unable to keep a scheduled appointment, we ask that you please notify
us at least 24 hours in advance, if possible.
Prescription Refills
If you need a prescription refilled, we ask that you call (608) 417-3434 and
leave a message. Please include the following information:
- Your name
- Your phone number
- Your date of birth

- Name of the pharmacy
- Amount and frequency of the medicine
|
We will call in your refill order to the pharmacy you designate within 24
hours, and notify you if there are circumstances that will not allow it to be
refilled.
Lab Work and Tests
Meriter Medical Clinic offers a full range of laboratory testing services on
site. If you need to stop in for lab tests between appointments, it is helpful
for you to call ahead of time so that we may prepare your lab orders. Unless
your physician states otherwise, we will contact you with any lab work results
that require follow up.
Hospitalist Care
If you should ever need to be admitted for hospitalization at Meriter
Hospital, you are apt to be under the care of hospitalists - UW Medical School
Faculty Physicians who specialize in the care of hospital patients. Hospitalists
work closely with your primary care physician and specialists to provide

Our Location
Our clinic is located in the heart of Middleton, at 7780 Elmwood Avenue,
Suite 201, just off University Avenue near the highway overpass.
Enter the main entrance off of the parking lot. Our clinic is on the second
floor, accessible by stairs or the elevator.
