As
the Madison area grows, there is an increased request for primary care services.
As part of Meriter's mission to serve the community, we currently offer two
primary care clinics to serve this need and a third clinic will open soon.
Beginning this summer, we are also broadening our services that are offered at
select Meriter Medical Clinic locations to include endocrinology, health
psychology, diabetes education and nutrition services.
Meriter Medical Clinic-Middleton
opened in 2004, and is staffed by six physicians board-certified in internal
medicine.
Meriter Medical Clinic-McKee is located near the intersection of Highways PD and M on the west side of Madison. The clinic is currently staffed by four physicians board-certified in internal medicine.
New Clinic Opening Soon!
Meriter Medical Clinic–West Washington will open August 31,
2009. This primary care clinic is conveniently located just off the capitol
square at 345 West Washington Avenue in Madison. More information on the clinic,
its providers and how to schedule an appointment will be available soon.

417-8300. Please note, appointment scheduling will not be handled through this
number until Monday, August 31.
A community open house for the new clinic will be held on Tuesday, August 25
from 5:30-7:00 p.m., so please join us to tour the facility and meet the
providers who will be practicing at this location.
Dr. Kutz, currently practicing at Meriter Medical Clinic–McKee, will be relocating to the West
Washington clinic upon opening. Dr. Kutz's patients can continue to call the
McKee clinic at (608) 417-8800 for appointments until August 31.

FastCare clinics offer basic medical care for patients 18 months and older. Treatment is provided for minor conditions and symptoms.
Transitioning Your Care to a Meriter Medical Clinic
If you would like to transition your care, you'll find helpful information in our
Transition of Care instructions.
Also, you'll need to send in an Authorization/ Consent for Release of Medical
Information form and a Patient Registration form. These forms can be found in
our Forms for New Patients section.
