Below is the brochure for Meriter's IRB Annual Seminar.

Meriter Hospital Institutional Review Board (IRB) is responsible for protecting the rights and welfare of the human subjects who participate in research conducted within Meriter facilities.

Meriter has an agreement with the federal government to comply with the code of federal regulations that protect human subjects. The agreement also binds Meriter to ethical principles described in the Belmont Report.

Meriter IRB considers federal and state laws as well as the Belmont Report when reviewing, approving, requiring modifications, or disapproving research.

Meriter and UW IRB Partnership

The Guidance will tell you:

• What the Meriter and UW-Madison IRB partnership is.
• Which studies fall under this agreement
• Which IRB is likely to serve as the IRB of record.
• How to initiate a request for consideration under the Meriter Hospital/UW-Madison IRB Partnership
• About scientific review requirements
• About human subject protection training requirements.

Next Steps Overview

If you will request Meriter to be your IRB of record:

1. Request an account in the online submission software.
2. Make sure you have complete the mandatory training on human subjects protection.
3. E-mail or fax your training completion transcript to the Meriter IRB Office.
4. Complete the deferral request in the online submission software.
5. You will need to attach the following documents to your deferral.
   • Meriter consent form (if applicable, see below).
   • Meriter HIPAA form (if applicable).
   • UW-Madison Conflict of Interest form signed by the Principal Investigator
   • UW-Madison Principal Investigator Status Policy and Request Form.
