Meriter Heart & Vascular’s Cardiovascular Rehabilitation program is an important element in your long-term recovery. During the program, you will work with Meriter cardiac rehabilitation staff to develop a personalized fitness plan that will help you rebuild your strength, endurance and emotional well-being.
Cardiovascular Rehabilitation is appropriate for people who have had a heart attack, bypass or valve surgery, angioplasty or stent. Meriter’s rehabilitation care team provides monitored exercise, nutrition, stress management and education about cardiovascular disease. Along with your cardiologist and primary care physician, you will work with Meriter dietitians, exercise physiologists, pharmacists and registered nurses in finding the most effective ways to reduce your risk for having another cardiac event.
Phase I - Inpatient Cardiac Rehabilitation
Our Phase I team are experts in cardiovascular disease prevention, treatment and recovery. They provide ECG-monitored exercise and individualized education for patients and family members related to heart and vascular disease and its risk factors.
Education includes how to manage high blood pressure and high cholesterol, smoking cessation and managing stress. The Phase I team also helps patients enroll in outpatient programs near their home and calls patients and their caregivers at home in the first few weeks following open heart surgery.
Phase II - Outpatient Cardiac Rehabilitation
Meriter’s Outpatient Cardiovascular Rehabilitation program fine tunes and builds upon the skills and life changes you began working on while you were in the hospital. Our program includes:
• Individualized plan to meet your long-term health goals and personal exercise needs.
• Risk factor reduction techniques that include:
- Comprehensive nutrition assessment and personal nutrition plan
- Cholesterol, blood pressure and diabetes management strategies
- Progressive exercise
- Weight management strategies, including a food tasting class and tips for dining out
- Smoking cessation
- Stress management
• Maintenance plan upon completion of rehabilitation program, including regular follow-up appointments with your exercise physiologist for continued progression.

Medicare and most private insurance companies reimburse a portion of the cost of cardiovascular rehabilitation. Some insurance companies may require prior physician authorization, which our staff can help you obtain.
To participate in Meriter’s Cardiovascular Rehabilitation program, you will need a medical referral. If you do not have a referral from your physician, our staff can help you obtain one.
