Meriter DeForest-Windsor is now open and accepting appointments and new patients!

This local clinic is now part of Meriter's new integrated health system that ultimately will make it easier for patients to see their family doctors. Community clinics like Meriter DeForest-Windsor allow Meriter Medical Group physicians to become even stronger partners in health with their patients. Our physicians' goal is to help people stay healthy and prevent disease, not simply work to help heal those who get sick or hurt.

We're proud to offer extended hours for our patients' convenience. Our doctors have flexible schedules and go the extra mile to make sure you get an appointment when you need it.

Monday - Thursday: 8 a.m. to 7 p.m.
Friday: 8 a.m. to 5 p.m.

Making Appointments

There is never a good time for an illness to arise or an injury to occur. Our staff strives to take the frustration out of appointment scheduling by accommodating your specific needs. We strive to stay on schedule, because we know how valuable your time is.

In order to be able to schedule ill patients on the same day they call the clinic, we hold open several appointment slots. If you feel you need to be seen on an urgent basis, we will likely ask you to speak with a clinical staff person who will assess your need and arrange for appropriate care.

Same Day Appointments - Same day appointments are available every day our clinic is open. Call early in the morning to schedule an appointment.

Road Construction Update

Please note, due to I-39/USH 51 structure replacements, there will be some traffic disruption as some of our patients travel to the DeForest clinic.

Apporximate project timing: March through November 2012

Major traffic concerns include:

-          I-39/USH 51 Interchange off ramps will be closed by April 15, 2012.

-          USH 51 will remain open, but at certain times may be narrowed to a single lane in each direction.

-          Traffic which needs to gain interstate access will be able to via STH 19 ramps.

Directions to Meriter DeForest-Windsor

The clinic is conveniently located on the southeast corner of Highways 19 & 51. If traveling eastbound on Highway 19, turn right onto Monticello Drive. If traveling westbound on Highway 19, turn left onto Williamsburg Way. Then make a right onto Savannah Drive to reach the clinic.

Please note that because this clinic is located in a new subdivision, you may not be able to access the address through an internet mapping site or GPS.

Meriter MyChart
Securely access your medical information online at your convenience. If you receive care at Meriter DeForest-Windsor and would like to access your medical information online, then click here to activate your MyChart account. After you make the request to activate your account, your personal activation code will be mailed to your home within 7 to 10 business days. Another option is to ask a clinic staff member to activate your account at your next clinic visit.

Online Bill Pay
You now have the option to pay your clinic bill online! Meriter’s online bill pay service allows you to securely submit payment for your health care bill in the most efficient manner possible – online with your credit card. To pay your clinic bill, go to meriter.com/billing.
