To make the coming weeks as easy as possible, we've included information below on employee orientation and the steps you need to take before your first day at work. In addition, you'll find copies of the new employment forms you need to fill out.

Steps You Need to Take BEFORE
Your First Day on the Job
1. Schedule a Pre-employment Physical with Employee Health All employees must be screened by Employee Health before beginning to work at Meriter. This includes a post-offer, pre-employment drug screening test. Your physical will vary depending on the amount of contact you will have with patients. To schedule your appointment, please contact Employee Health at (608) 417-6158. After your appointment they will remind you to visit Human Resources to hand in your pre-employment forms.
2. Fill Out Your Pre-employment Forms
You should print out your pre-employment forms, fill them in and bring them to Human Resources.
Note that this file includes:
- Federal W-4 Form - for tax purposes
- Wisconsin Withholding Exemption Certificate
- Department of Justice Employment Eligibility Verification Form (I-9) – Note that you need to show two forms of identification when handing in this form. Please read the form for the types of identification that can be used
- Department of Health and Family Services Background Information Disclosure Form - Note that this form is only required for select positions. To determine if you need to fill it out, please call Human Resources at (608) 417-6134
- Authority to Release Information, Out of State Criminal History Search Form - Note that this form is only required for select positions. To determine if you need to fill it out, please call Human Resources at (608) 417-6134
- Direct Deposit Authorization Agreement

3. Have Your Employee Photo Taken
After going to Human Resources, you can have your employee photo taken in Guest Services in the lobby. Photo services are available 10 am – 2 pm, Monday-Friday. If you are unable to visit Guest Services during these times, please contact Renee Boyd at (608) 417-6276 to make arrangements.
4. Receive Your Parking Assignment
After you've had your photo taken, please talk to Guest Services about your parking assignment.
Information on New Employee Orientation
We provide hospital-wide orientations for new employees to help them learn about Meriter.
You will be scheduled for orientation either on your first day of the job, or in the first month of employment. You should have received a schedule and information about your orientation prior to your first day of work. If not, please call Human Resources at (608) 417-6134.
Please make sure to contact your supervisor regarding your work schedule before you attend this orientation training. Also, if you need any special accommodations for this orientation, please contact Human Resources at (608) 417-6134.
Please note the following about orientation:
- Orientation begins promptly - please allow enough time for finding your new parking assignment and using the shuttle if necessary.
- Note that lunch is provided as part of the orientation program.
- Dress for orientation is business casual.
Orientation is held in the Community Health Education Center. The Center is located in Meriter Hospital (202 S. Park Street) on the second floor of the Atrium. For driving directions to Meriter, please visit the Visitor Information section.
